Issue. On certain vehicles, the front steering knuckle(s) may not have been manufactured properly. As a result, the knuckle(s) could break and cause a loss of steering control.

Safety risk. A loss of steering control could create the risk of a crash.

Corrective action. Lexus will notify owners by mail and advise you to take your vehicle to a dealership to inspect and, if necessary, replace the steering knuckle(s).

Issue. Under certain conditions, a software problem could cause the rearview camera image not to display when the transmission is shifted into reverse.

Safety risk. A rearview camera image that doesn't display could reduce a driver's ability to see behind the vehicle while backing up. This could increase the risk of a crash.

Corrective action. Lexus will notify owners by mail and advise you to take your vehicle to a dealership to update the rearview camera software or replace the rearview camera, as necessary.

Issue. Under certain conditions, a software problem could cause the rearview camera image to freeze or not to display. If this happens, the rearview image may not display when the transmission is shifted into reverse. Canadian regulations require the rearview image to display when the vehicle is reversing. Note: This recall only affects certain vehicles equipped with a panoramic view monitor system.

Safety risk. A rearview camera image that doesn't display correctly could reduce a driver's ability to see behind the vehicle while backing up. This could increase the risk of a crash.

Corrective action. The company will notify owners in writing and advise you to take your vehicle to a dealership to update the parking assist ECU software.

Issue. On certain vehicles, a software problem could cause the combination meter in the instrument panel not to display when the vehicle is started. As a result, you may not be able to see certain information about the vehicle, including the speedometer, fuel gauge, warning lights and malfunction tell-tales. Note: This recall only affects vehicles equipped with a 12.3-inch display.

Safety risk. Warnings, tell-tales and gauges that don't display could increase the risk of a crash.

Corrective action. The company will notify owners by mail and advise you to take your vehicle to a dealership to update the combination meter software. Alternatively, certain eligible vehicles will be sent a wireless over-the-air software update, which you can install yourself. For certain vehicles, the dealer will also inspect and, if necessary, replace the combination meter.

Issue. On certain vehicles, the second-row center seat belt may have been damaged during production. As a result, the seat belt could break in a crash. If this happens, the seat belt may not hold a person in place during the crash.

Safety risk. A seat belt that doesn't work properly could increase the risk of injury in a crash.

Corrective action. To reduce the safety risk, the company advises that you shouldn't transport a passenger in the second-row center seat until the recall repairs are completed. The company will notify owners by mail and advise you to take your vehicle to a dealership to inspect and, if necessary, replace the second-row center seat belt assembly.

Issue. On certain vehicles, the head restraints on the front seats may not have been manufactured properly. As a result, the head restraints could move or detach from the seats in a crash.

Safety risk. Head restraints that move or detach in a crash could increase the risk of an injury.

Corrective action. Lexus will notify owners by mail and advise you to take your vehicle to a dealership to replace the front seat head restraints.

Issue. On certain vehicles, a sensor for the occupant classification system (OCS) may not have been manufactured properly. As a result, the sensor could short circuit and cause the airbag system to turn off the passenger-front airbag even when a passenger is seated. Note: This problem would cause a warning message to display on the instrument panel and the airbag indicator light would display "passenger airbag off".

Safety risk. An airbag that is turned off could increase the risk of injury to a passenger in a crash.

Corrective action. Toyota will notify owners by mail and advise you to take your vehicle to a dealership to inspect and, if necessary, replace the OCS sensor.

Issue. This recall is for certain block heater kits installed on certain vehicles or sold as accessories at Toyota or Lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. If this happens, the cable could short circuit while plugged in, and cause the wiring to overheat or melt. Corrective action. Toyota recommends that you should stop using the engine block heater immediately. Toyota will notify owners by mail and instruct you to take your vehicle to a dealer to disable the engine block heater. For some vehicles, the corrective actions for this recall are under development. For others, a new engine block heater cannot be installed, and the owner will receive a refund of the purchase price of the engine block heater.

Issue. On certain vehicles, the low-pressure fuel pump could fail. If this happens, then engine may run rough or may not start and the check engine light may turn on. This could also result in a sudden loss of engine power while driving. Note: This recall is an expansion of recalls SRC R95/SRC R96 and SRC RA2/SRC RA3 (Transport Canada Recall Numbers 2020-005 and 2020-088).

Issue. On certain vehicles, an oil passage in the rotor of the vacuum pump may not have been machined properly and could result in oil not being distributed throughout the pump. This could lead to the illumination of a warning light and message, an audible tone, and a sudden loss of braking assist, which could increase stopping distances and increase the risk of a crash causing injury and/or damage to property.

Corrective action. Dealers will replace the vacuum pump with a new one.

Issue. On certain vehicles, the airbag system pressure sensors and/or acceleration sensors used to detect vehicle impact may contain manufacturing defects. As a result, the airbag warning light may illuminate and the side/curtain shield airbags and/or front airbags may not deploy, increasing the risk of injury in the event of a crash.

Corrective action. Dealers will inspect and replace the defective sensors as necessary. Note: This will not affect the front airbags of the Prius model.

Issue. On certain vehicles, the electrical power cords of some dealer installed block heaters may have been improperly manufactured, causing the wires to contact each other, resulting in a short circuit. A short circuit could lead to a fire which would increase the risk of injury and/or damage to property.

Corrective action. For vehicles where the subject block heater was approved for installation, the dealer will replace the block heater assembly with a new one of an improved design. For vehicles where the subject block heater was not approved for installation, dealers will remove the block heater assembly. In the interim, until the remedy can be performed, dealers will cut the plug end from the block heater cord to disable the block heater's operation.

Issue. Certain vehicles may not comply with Canada Motor Vehicle Safety Standard 302 - Flammability. Some of the materials used in the construction of the seat upholstery may not meet the requirements of the standard when tested individually. The seat does meet the requirements of the standard when tested as an assembly.

Corrective action. A production change has been undertaken to remedy this non-compliance condition. No corrective recall action is required as the non-compliance condition is deemed to be non-safety related.

Issue. Certain vehicles fail to comply with the requirements of Canada Motor Vehicle Safety Standard (CMVSS) 126 - Electronic Stability Control Systems. Some brake actuator pumps may have been assembled with an O-ring that is damaged in such a way that the brake fluid pressure may not be properly controlled during Anti-Lock Braking (ABS), Traction Control (TRAC), and Vehicle Stability Control (VSC) system activation(s), which fails to meet the requirements of the standard. This could result in a loss of vehicle stability control, which could increase the risk of a crash causing injury and/or damage to property.

Corrective action. Dealers will inspect the serial number of the brake actuator and, if necessary, replace the brake actuator assembly.

Issue. This recall is for certain block heater kits installed on certain vehicles or sold as accessories at Toyota or Lexus dealerships. On certain vehicles, the engine block heater cable may not have enough heat protection and could be damaged by high temperatures. Issue. On certain vehicles, the end cap on the right-hand side fuel delivery pipe, located in the engine compartment, could have been improperly welded at time of manufacture. This could result in a fuel leak which, in the presence of an ignition source, could result in a fire causing injury and/or property damage.

Corrective action. Dealers will inspect the fuel delivery pipe and replace with a revised part if necessary.

Issue. On certain vehicles, a manufacturing defect could affect the functioning of the electric brake actuator, illuminating various warning lights in the instrument panel of the vehicle and disabling the antilock braking system (ABS), traction control and vehicle stability control systems. This could increase the risk of a crash causing injury and/or property damage.

Corrective action. Dealers will update the vehicle stability control ECU software and affect repairs as necessary.

Issue. On certain vehicles, the accelerator pedal may become stuck in the wide open position due to an unsecured or incompatible driver’s floor mat. A fully depressed accelerator pedal may result in very high vehicle speeds and make it difficult to slow or stop the vehicle. This could result in a crash causing property damage and/or personal injury.

Corrective action. Dealers will modify or replace the accelerator pedal.

Issue. On certain vehicles equipped with Vehicle Dynamics Integrated Management (VDIM), the calibration for the hydraulic control valves was incorrectly written and programmed into the brake actuator electronic control unit. As such, the right front wheel could lock during Anti-lock Brake System (ABS) or Vehicle Stability Control (VSC) actuation. This could result in increased stopping distances and/or failure to adequately correct excessive vehicle yaw, which could result in a crash causing property damage and/or personal injury.

Corrective action. Dealers will replace the brake actuator assembly with an updated version.

This could cause various warning lamps, including the malfunction indicator lamp, slip indicator lamp, brake system warning lamp, and master warning lamp, to illuminate on the instrument panel. If this occurred, in most cases, the vehicle would enter fail-safe driving mode, resulting in reduced motive power in which the vehicle could still be driven for short distances. In some instances, the fuse of the power supply circuit could blow, causing the hybrid system to stop functioning. The resultant loss of vehicle propulsion, in conjunction with traffic and road conditions, and the driver's reactions, could increase the risk of a crash causing property damage and/or personal injury.

Corrective action. Dealers will inspect and, if necessary, replace the Intelligent Power Module.

Issue. On certain vehicles equipped with a 1MZ-FE or 3MZ-FE V6 engine, the crankshaft pulley may fail. The outer ring may become misaligned, or it may not properly rotate, causing noise and/or illumination of the discharge warning light. In addition, the power steering belt may become detached from the crankshaft pulley, which could result in a sudden increase in steering effort. A loss of steering power assist could result in a crash causing property damage and/or personal injury.

Corrective action. Dealers will inspect and, if necessary, replace the crankshaft pulley.

Issue. On certain vehicles, a control board for the Intelligent Power Module (IPM), part of the hybrid system inverter assembly, could fail while the vehicle is underway. Issue. On certain vehicles, if the forward retention clip used to secure the floor carpet cover, which is located in front of the center console, is not installed properly, the cover may lean toward the accelerator pedal and interfere with the operation of the accelerator pedal arm. If this occurs, the accelerator pedal could temporarily become stuck in a partially depressed position, rather than return to the idle position. This could result in a loss of throttle control and a crash causing property damage and/or personal injury.

Corrective action. Dealers will replace the floor carpet cover with a newly designed version. Note: This campaign also covers vehicles which may have been previously included in recall 2006168 (Toyota 609).

Issue. Certain vehicles use brake fluids containing polymers that act as lubricants for certain brake system components. If during vehicle maintenance, brake fluid is used that does not contain such polymers or only small amounts, part of the internal rubber seal located at the end of the brake master cylinder piston may become dry and may curl during movement of the piston. If this occurs, a small amount of the brake fluid could slowly leak from the brake master cylinder into the brake booster, resulting in illumination of the brake warning lamp. If the brake warning light has illuminated and the vehicle continues to be operated without refilling the master cylinder brake fluid reservoir, the driver will begin to notice a spongy or soft brake pedal feel and braking performance may gradually decline. This will increase the stopping distance, increasing the likelihood of a crash.

Corrective action. Dealers will replace the brake master cylinder seal with a newly designed one.

Issue. On certain vehicles, due to the improper installation of the retaining clips for the floor carpet cover located in front of the center console, there is a possibility that the clips may become loose. If both clips separate from the floor carpet cover, the cover may lean toward the accelerator pedal, causing interference with the accelerator pedal rod. In the worst case, the accelerator pedal may become stuck in a partially depressed position when returning to the idle position.

Corrective action. Dealers will install new clips.

Issue. Certain vehicles do not comply with the requirements of CMVSS 210. The left rear seat belt buckle anchor may separate from the floor before holding the required test load for the required time. If a separation occurred in a crash, the left rear seat occupant may not be properly restrained, increasing the risk of personal injury.

Corrective action. Dealers will replace the left rear seat belt buckle anchor bolt.

Issue. On certain vehicles equipped with a light emitting diode (LED) stop lamp, there is a possibility that silicon oxide may be generated on the surface of the contact point inside the stop lamp switch. Consequently, current may not flow at the contact, causing the stop lamp to become inoperative. The loss of stop lamps would result in failure to warn following drivers that the vehicle is braking and/or is stopped, which could lead to a vehicle crash.

Corrective action. Dealers will replace the stop lamp switch.
